A child can have more than one over time. - Full Case History form — each therapy record now has a complete case-history proforma covering Family History, Prenatal/Perinatal/Postnatal, Oro-motor, Behavioural, Hearing Assessment, Schooling, Documents, and Diagnosis & Logistics.

UI bugs found - 1. Full page reload logs the user out (no silent refresh) 2. Mobile "More" sheet not dismissible via Escape / re-tap 3. Em-dash tail in Social Worker copy Api (not really a bug) 1. On this endpoint - /children/:badUuid if Invalid UUID path param is passed than expected status code should be 400 (bad id format) or 404 (not found) but it returns 500
