implement repeat query detection

Implement repeat-query detection so that if a user asks the same question again, the bot can either rephrase the answer, ask a clarifying question, or intelligently escalate to a human/contact option instead of restarting the same process. - Check how we make use of system_note and see if this can be solved using that.
